Policy and Waiver

• No additional equipment is required—I only need a space large enough to move around comfortably.
• Respect  and   professionalism  are expected   at all times.
• Booking Process:  All appointments must be booked online. You are required to complete the intake form and review & sign the policy and waiver form.   Please enter the service location in the “Address” field.
• Service Location:  If your chosen location is outside of your home, please ensure it is a venue that allows massage services.
• Appointment Time:  The time slot you book is final. Extensions or additional minutes cannot be accommodated, as I respect the schedules of all clients throughout the day.

Arrival, Late Arrival, No-Call No-Show, Cancellations & Charges

•Arrival:   Please plan to arrive  5-10 minutes before   your scheduled appointment. This allows time to settle in so you can enjoy your full session without feeling rushed.

 •  Late Arrival:   Life happens! If you’re running late, I’ll do my best to accommodate you; however, your session will still end at the scheduled time to respect other clients’ appointments.

 •  Excessive Lateness:   If you’re more than 15 minutes late, we may need to reschedule, and the appointment will be considered a missed session. Refunds cannot be provided in these cases.

 •  No-Call No-Show:   If you’re unable to make it, please let me know as soon as possible. No-call, no-shows are  non-refundable.

 •  Cancellations & Rescheduling:  If you need to cancel or reschedule, please give at least 24 hours’ notice so we can find a better time for you!

I, the undersigned, acknowledge that the massage services provided by Molly's Massage Heaven are for stress reduction and relaxation only, and are non-sexual in nature. I understand that these services are not a substitute for medical care, and that my provider is not a licensed massage therapist and is not qualified to diagnose, treat, or prescribe for any medical or psychological conditions.

If I experience discomfort or pain during the session, I agree to inform my provider immediately so that adjustments can be made for my comfort. I voluntarily assume all risks related to the massage and will not hold my provider or Molly's Massage Heaven responsible for any discomfort or pain that may occur.

I affirm that I have disclosed all known medical conditions and injuries to my provider and will update them on any health changes prior to future appointments. I agree to maintain a respectful attitude during the session. I understand that loud noise and disrespectful behavior may be disruptive and will be addressed accordingly, and may result in the termination of services without a refund and prevent me from rebooking.

By signing this waiver, I release, discharge, and hold harmless Molly's Massage Heaven, its owners, employees, agents, and affiliates from any and all claims, liabilities, or damages arising out of or in connection with the services provided. I understand and agree that this release applies to both known and unknown risks.
